You will be working with an expanding company that specialises in providing support to clients with mental health/Learning Disabilities/Dual Diagnosis problems.
You will assist with the planning and co-ordination of the delivery and support services.
Ensure that clients get opportunities to exercise determination and choice to participate in the wider community.
Write support plans and risk assessments.
Work to a shift pattern as required.
Monitor, or prompt, or where appropriate administer medication.
Assist clients with personal budgets
Participate in meetings and training when required.
As this is a community based role, you must have your own transport.
An enhanced DBS is required.
